Speakers at Nigeria's 2026 Health Equity Conference said weak coordination between hospitals, laboratories, pharmacies and specialists often leaves patients to find their own way through the health system, Premium Times reports.
Patients and relatives frequently carry medical records between facilities and have to repeat their histories to each new provider, delegates heard. Yemi Ajao, chief executive of OneBarrow International, said information and responsibility for a patient's care should travel with them when they are referred elsewhere, and that someone should check each referral is completed.
The conference, held from 13 to 15 October, focused on care coordination and brought together government officials, health institutions and professional bodies. Speakers called for better referral systems, clearer responsibilities and closer cooperation between public agencies, regulators and private providers.
